Basic Elements of Effective Skincare

For the past two years, I have really taken a serious approach towards researching and understanding the primary factors relating to effective skincare.  What works and why it works is my main focus.  So, within the next few blogs, I want to address some critical points to help you understand and hopefully, establish an effective pathway towards creating and maintaining your beautiful complexion, including your neck and declotte.

Let's Start With Some Basics

To quote Dr. Ben Johnson, "Circulation has everything to do with your skin's health and it is substantially overlooked by the skincare community.  All the skin's food, immune support, antioxidant and remodeling efforts come from the blood supply.  Every time you are stressed, you drink coffee or go outside in cold weather, your skin suffers a little because all of theses things restrict bloodflow to the dermis.  Vitamin K, horse chestnut and caffeine in skincare products are all designed to reduce bloodflow.  There is no skin condition (Rosacea, Dark Circles, etc) that is better off with less bloodflow.  That is why the utilization of Retinaldehyde, Niacinamide, Camphor, and other circulation enhancing ingredients make such a big difference in the quality and health of the skin.  Bags under the eyes are the result of vasoconstriction (reduced bloodflow and the correlation between loss of blood supply and thinning of the dermis with are are more than a coincidence.)  remember, it is a thinning dermis that is the primary cause of visible capillaries.  The reason capillaries return after treatment is because the skin wants adequate blood supply in the area.  The best way to reduce this is to build back the collagen/elastin (in the dermis) that covers these vessels.

Sunscreens Exposed

Summer is almost here and in Texas, the sun is beating down upon us as I write this little blog.  In the interest of your health, here is the scoop on Sunscreens.  We do know that most prevent sunburns, but beyond that simple fact many of us do not realize the safety and efficacy of these ubiquitous creams and sprays.

Here are some surprising facts about sunscreens:

·        There is evidence that some sunscreens might increase the risk of the deadliest form of skin cancer for some people.   One factor that scientists feel may be causing this reaction is the fact that some of the chemicals break down in sunlight and may play a role in creating cancer causing free radicals on the skin.  Furthermore, there are some products that have dominated the sunscreen market that have poor UVA protection that may have also led to this surprising outcome!  Inferior sunscreens with poor UVA protection may give the consumer the impression that they may stay out in the sun longer but they end up absorbing more of the UVA rays which are the most harmful type of sun exposure.

·         Another concern is that some sunscreens may contain Vitamin A that could speed the development of cancer.  Recently, available data from an FDA study indicate that a form of vitamin A, retinyl palmitate, when applied to the skin in the presence of sunlight, may speed the development of skin tumors and lesions.  This evidence is troubling because the sunscreen industry adds vitamin A to 30% of all sunscreens.
·      
     The Environmental Working Group (EWG) has determined that mineral sunscreens have the best safety profile of today’s choices.  They are stable in sunlight and do not appear to penetrate the skin as do chemical sunscreens.  They offer uva/uvb protection, which is sorely lacking in most of today’s sunscreen products.
     
     Chemical Warning - Presently, scientists have called for parents to avoid using oxybenzone on children due to penetration and toxicity concerns. Of course, with that stated, we should all avoid oxybenzone.

Adding to the confusion:  Too little sun might be harmful, reducing the body’s vitamin D levels.

It is a known fact that sunshine serves a critical function in the body that sunscreen appears to inhibit – production of vitamin D.  The main source of vitamin D in the body is sunshine, which is enormously important to health – it strengthens bones and the immune system, reduces the risk of various cancers (including breast, colon, kidney and ovarian cancers) and regulates at least 1,000 different genes governing virtually every tissue in the body (Mead 2008).  According to scientists, vitamin D levels have been steadily decreasing within the U.S. population creating a “growing epidemic of vitamin D insufficiency (Glinde2009a).  Seven of every 10 U.S. children now have low levels.

Effective Anti-aging Strategies for Your Skin

Finally, a few minutes to sit down to write my little blog to stay in touch and also to educate you on some vital information concerning our skin.  Today, we are so fortunate to live in an age of great scientific discovery and research into why our skin ages and what to do to slow down this process.
Why does our skin age?
Unraveling the mystery of the human aging process has been one of the biggest research topics in skin care the past few years.  Research has discovered that one of the biggest reasons for our continual aging process, both within our bodies and the visible effects of aging skin are free radicals.  Free radicals come from our diets, stress, sun and environmental pollutants which all cause cellular death and aging.  As our skin increases its free radical load, the more damage to our skin is visible.

Having a deeper understanding of the instigators and processes by which skin develops challenging conditions or degrades with visible aging will continue to fuel research and the discovery of topical agents to address this.  Also, effective skin care regimens that are consistent with the findings of pro-active skin care focus on the improvement of barrier functions (antioxidants), promotion of normal desquamation, exfoliation (skin turnover) and UV protection.

Rosacea - Sensitive Skin

Today, I would like to discuss Rosacea with you.  Generally, Rosacea is a condition in which your
cheeks and nose area become red and inflammed and often pitted with pimples and broken capillaries.  Left alone, this condition could result in scarring within your dermal tissue even to the point of advanced Rhinophyma which is what happened to
W.C. Fields and his bulbous nose.

We are presented with clear evidence that "something" is causing the skin to be inflamed and it is the strong research findings of Dr. Ben Johnson that most triggers are related to digestion/foods.  Some clinicians argue that exercise and environmental changes are triggers; however, these factors could cause further excitation of the existing inflamed state of the skin but are not the sole problem.

Rosacea causes the skin to thin much faster than the normal aging process which then exposes capillaries and visible signs of Telangetasia are present.

Let's Consider Calming and Repairing Compromised Rosacea Skin

Individuals that suffer with Rosacea generally have red and inflammed cheeks and often their nose is also involved.  Most of the redness we see is a normal repair response of our body to correct the damage...that is a good thing.   According to Dr. Johnson, "many treating physicians and skin care professionals place the blame on the vascular system and thus collapse these healing vessels with lasers."  Initially, one may think that would be a good thing.  However, when you think about our body and the gift of our vascular system, it's purpose is to feed the tissue with antioxidants, enzmes, proteins and lips to maintain a beautiful complexion.  So, if we destroy the capillaries that are feeding our tissue, we seem to be defeating the purpose of healing the Rosacea in the long run.

Treating Abnormal Pigmentation

Are You Speckled Like a Brown Spotted Egg?

Hyperpigmentation, an umbrella term for sun spots,melasma, age and brown/liver spots, is a major skin woe for women.  In lay terms, hyperpigmentation is an over-production of melanin, usually from cumulative over-exposure to the sun.  If you spit-roasted in your twenties, sun spots - as we'll call them all - are almost a given.  Melasma is another type of hyperpigmentation usually triggered by pregnancy, the use of oral contraceptives, or in some women by hormone replacement.

Whatever the cause------ we don't like them!!

Since beginning my skin care practice, I have tried many products and procedures that are proclaimed to help rid us of these unwanted spots on our face, neck, declotte and arms.  Lasers will take away the spots - they actually flake off.  However, given any sun exposure....they're back, haunting us again.  Abnormal pigment can be one of the most stubborn and frustrating skin disorders to not only treat, but to live with as well - trust me - I know this from very personal experience and have watched these little spots grow as my years progress.

(The photo to the right is of a very pretty young woman with freckles...it was the closest I could find to illustrate brown spots on the skin.... most of us over the age of 45 or so don't have such pretty spots!)

Camouflage make-up has been used, if you don't mind heavy makeup.  Safer lightening creams with ingredients such as fruit acids (AHAs and BHAs) may help to reduce the sun spots, exfoliating the top skin layers while a bleaching agent, such as hydroquinone,, lightens the newly forming cells.  The hydroquinone that has been most effective has been in high-dose creams available only through dermatologists.  However, it can take two or three months of daily application for color to fade (only fade....what does that tell you??? Of course, sun exposure and it's back!)  Hydroquinone has downsides, too.    It actually kills the skin cells!

Is The Person Working On You a Trained Technician???


We can thank our lucky stars  we were born in the 20th Century.  Once upon a time, wrinkles were an inevitable fact of life.  Don't get me wrong: some lines we love.  The only way not to get laugh lines is to never laugh...But today, we can all beam about the fact that skincare is up there with rocket science, searching for ways to keep time at bay and even turn back the clock!!

As I write my blog every couple weeks, I will try to focus upon the newest and latest skin care products, procedures and facts to help you in your quest for beautiful skin.
While I could go on for pages in regard to how beautiful your skin will look after a couple Fraxel treatments, or how nice it would be to have the hair on your legs removed by laser or your unslightly brown spots eliminated....that is not my focus today.

Are the Providers You Use for Laser Procedures Thoroughly Trained?
I broach this subject because it concerns me that many "med spa's" are using untrained Estheticians and/or medical professionals to perform Laser/IPL skin care procedures.  The spa may say that "they have trained their Estheticians to perform laser services".....  Oh, is that so?  They may have been trained by the saleman that sold the spa their laser, for a couple hours.  Not enough to ensure your safety and the ultimate outcome of your procedure!!  What type of training did the Dr. have .... was he also trained by the salesman for a couple hours?  If you are considering laser procedures, be sure to do you due diligence and make sure the individual that owns  and operates the clinic is either a Plastic Surgeon or Dermatologist. These docs understand the nature of the largest organ in our body (our skin).  They are highly quaified on lasers, understand the physiology of the skin and also understand the importance of hiring certified laser technicians to either assist them or perform the laser procedures.

As most of you know, I attended the National Laser Institute to learn the physics as well as the techniques for using lasers to perform procedures like Laser Hair Removal, Fraxels and IPL treatments.  While in school, there was a tremendous amount of of emphasis placed upon us to learn the physics of the lasers, understanding the Fitzpatrick of our clients as well as hereditary influences, whether the client had a suntan or sunburn and any medications they may be taking that would influence the outcome of the procedure.   There are many factors that we must consider before placing that laser on our clients body.  We were trained to understand the critical nature of the starting fluence and the pulse durations...all critical factors that must be taken into consideration.  There is no guessing game here when it comes to your safety and beautiful face.  Laser safety hazards to the skin can include burns which can in turn cause hypopigmentation and hyperpigmentation and painful and severe skin damage. 

Before you make the decision to have any type of laser service, please make sure you find out exactly what type of training the Technicians have received.  If they have not attended at least two solid weeks or more of school like the National Laser Institute and are "Certified Laser Safety Officers," I would say to pass on that facility.  It is just not worth the risk!!  I don't care if the clinic says they have performed lots of these procedures, you must consider this as your due diligence and the risks are just not worth the outcome with less than trained technicians.

In the past month alone, I have heard from clients that have experienced terrible outcomes from laser procedures. The clients went in for simple hair removal procedures and the untrained technicians actually fired the laser over a tattoo resulting in deep burns on the clients leg that required antibiotics and weeks of healing and an ugly scar on their leg.  Another technician fired the laser at very high fluence settings and burned holes in the clients face.   A tennis instructor went in for a fraxel ...he had a suntan as well as a sunburn on top of that, but the technician went ahead and fired the laser on the advice of the Dr. and the client received third degree burns which ultimately resulted in hypopigmentation (loss of skin color that will never again have melanin or color).  This is just an example of the stories I have heard and the damaged skin that I have seen as a result of untrained technicians.

The photo on the left is of a patient receiving a Fraxel treatment.  The laser actually makes hundreds of pinpoint holes on the face.  The procedure is somewhat painful, however, your doctor or technician will generally numb your face with a topical cream prior to starting.  Your face will be very red and may feel like a bad sunburn for about a week.  It is critical that you receive pre and post Fraxel consultation and care.  This is a very general description of what you may expect, however, the results are outstanding.  After a few weeks you will see dramatic changes in your skin and face.

As you can see from the photo on the right, this is
a laser procedure that has gone bad.  The technician
fired the laser at too high fluence and the pulse duration was probably too fast for her skin Fitzpatrick.  She will have these red mark tracks for some time and then they will probably cause some hyperpigmentation on her skin.  Of course, this is mild in comparison to some mistakes made with lasers.  She at least does not have severe burns or holes that will result in scars.




The photo on the left may actually give you
a better understanding of just what can happen if you are treated by an untrained technician... This woman has burns and track marks on her legs that will be rather painful and will more than likely cause brown spots on her legs which may fade with time...maybe.
This was really sloppy work.  The power used was much to high, the entry speed to fast and the power much too high for her skin type and and there was no care in the proper overlapping of the laser beam so you know the technician was not properly trained.  Ouch!!


Your skin is the largest organ of your body!!  If you are considering Laser procedures, tread with caution and investigate the facility and anyone that is scheduled to perform your procedure.

It's Time For School - Are Your Teens Ready?

If your teens are going through these early adolescent years with slight blemishes that tend to make them feel less than confident, why not help them achieve clear, healthy skin this school year.  This can be done through a series of express treatments specifically geared for teens along with a very simple home care regimen.
